As our Healthcare Facilitator you will be promoting the health and wellbeing for people supported by Achieve together, advising and training support teams – with emphasis on promotion, prevention and proactive planning
You will also ensure excellent research, evidenced based support and advice related to physical health issues, mobility & postural care, nutrition & hydration, infection prevention and control, sexual wellbeing, health screening and end of life care
Key Responsibilities…
- Provide effective health prevention and promotion advice, information and training to support teams in keeping with the organisation health strategy
- Advise managers on hospital admissions and discharges, to assist in ensuring people receive relevant investigations and treatment, avoiding premature discharge from hospital
- Advise services on good nutrition and hydration for wellbeing
- Assist in advising support teams in ageing health issues, including dementia support
- Ensuring effective oversight of health support to services supporting people with profound and multiple learning disabilities – focused on the PMLD Core & Essential Service Standards
- Assist in advising support teams on effective end of life care

Experience required…
- Achieved a minimum of Level 3 QCF
- Skilled at advising and supporting teams to ensure effective health and wellbeing support to adults with learning disabilities
- Skilled at engaging with a range of health professionals to ensure effective multi-professional support as required
- Able to use autonomy and work creatively to share information and advice across the organisation, prioritising workload effectively
- Skilled at supporting the holistic needs of people with profound and multiple learning disabilities
Key Competencies…
- To engage professionally and respectfully with others, whilst advocating for excellent health and wellbeing for people supported by the organisation
- Able to work proactively with other internal departments and external health/social care professionals to promote the organisation and collaborative partnership working
- Able to assess the competency of managers and support teams with specialist healthcare procedures
- Excellent verbal, written and IT skills to ensure accurate and professional records and reports are developed as appropriate
