Complex Clinical Case Manager- Staffordshire
Full time Permanent (37.5 hours per week)
£48k per annum + car allowance
Location: Shropshire area
Role Overview
Reporting to the Head of Operations the Complex Case Manager will provide a confident, knowledgeable, and efficient front-line service to people supported and management teams. Supporting a collaborative process of coordinating individual care through planning, implementation, and evaluation of needs. Ensuring focus is applied to guiding people supported towards goals, communication, and timely interventions when the path deviates. Working collaboratively with external stakeholders and clinical professionals.
- Support and contribute to the multi-disciplinary team and our wider health team meetings.
- Ensure to promote inclusion for people supported’ s voice to be heard.
- Ensure cultural, social, physical, functional, and medical needs for all people are being met in accordance to Care plans and Health Action plans.
- To contribute to effective clinical, and operational performance within the region with proactive clinical intervention working in conjunction with externally clinical teams.
- Operating within a CQC framework and legislation.
- Undertake the role of clinical coordinator and case-manage in partnership with external responsible clinicians and commissioning teams.
- Proactively guiding teams to maximise best outcomes and experience ensuring care is appropriate to need and in accordance to Positive Behaviour support plan/care needs assessments/HAP.
- Proactively manage queries and work in collaboration with the clinical teams responsible for commissioning the care and support
- Work proactively to identify areas of risk to ensure proactive measures are in place to reduce risks of hospital admissions associated to escalation of behaviours.
- Maintain up to date and accurate knowledge of current best practice and clinical guidelines to ensure all decisions are appropriately made in conjunction with clinical & commissioning teams.
- Use clinical skills to make decisions to guide and mentor teams around effective support in accordance with effective care and support plans.
- Utilise Achieve Together systems as required to maintain accurate updated information and record keeping.
- Facilitate workshops to educate, couch and mentor teams around the legal framework and clinical responsibilities associated to the care delivered.
- Escalation of concerns associated to clinical risk to Head of operations, internal quality teams and external stakeholders .
- Participate In assessments and transition work to ensure effective measures are in place to manage associated clinical risks for new placements.
What You'll Need
- Registered Mental Health Nurse/Registered learning disability nurse.
- Clinical experience across a range of clinical areas.
- Previous complex case manager experience.
- Previous learning disability experience.
- Ability to deal calmly and professionally face to face or remotely with professionals at all levels managing complex and challenging conversations.
- Resilient and able to keep difficult conversations and challenges in perspective in a pressured environment particularly where there are high emotions and/or vulnerable people.
- Ability to identify and be proactive in acting to quickly resolve problems, taking ownership to ensure speedy resolution and communication.
- Ability to make decisions based on the information gathered, within the guidelines/protocols set.
